Welcome. If you are seeking existing patient information, you've come to the right place. Below are several forms, which should be reviewed/filled out if necessary and either mailed/faxed back to our office.  Other general information is offered below as well.  If you still have any further questions, please do not hesitate to call our office.

Communication is key here at Bakersfield Neuroscience.  You're problems, questions, and concerns are our number one priority. If you want to contact a physician from our practice after office hours, call the practice's main phone number, (661)872-9999 and leave a message with the answering service.  The answering service will contact the physician who is on call and relay your message.  If you require immediate medical attention, call 911 or proceed to an emergency room at your local hospital.  If you are an established patient with us, the emergency room staff will contact our physician on call.

X-Rays & MRI

 

Patients are responsible to hand-carry all relevant radiological studies personally. These studies or films must be picked up from the facility at which they were conducted.
Please make arrangements well in advance of your scheduling appointment.
